Elite Waiver Form

 

Once a player is released, request that the resident club submit AAA Elite Waiver form on the player’s behalf.

The resident club will then fill out and submit Hockey Alberta AAA form:

Hockey Alberta AAA Elite Waiver Form

Any player release will be approved and authorized by the resident club.
The parent does not fill out the Hockey Alberta form but can make the request to the resident club within 3 days of release.
